Services for Adults

CLH Developmental Support Services offers 7 programs/services for adults with developmental support needs:

  • Accommodations – Supported Independent Living, Supported Living Homes, Host Family Provider
  • Community – Community Participation Supports, Passport Program
  • Employment – Supported Employment Program
  • Coordination (for those with high support or complex care needs) – Community Networks of Specialized Care- Central East

All programs/services listed above are provided within the North Simcoe Community, except for the CNSC-CE, these services span all of Simcoe County, York, Durham, Haliburton, Kawartha, and Pine Ridge.

Developmental Services Ontario (DSO) is the access point for adult developmental services funded by the Ministry of Children, Community and Social Services (MCCSS) in Ontario. To access Ministry funded services at CLH DSS please call 1-855-277-2121 or link to their website below.

Programs for Adults

SEP assists individuals 18 years of age and over explore/obtain and/or maintain employment opportunities.

CPS is a support service providing individuals with a broad range of social and recreational opportunities in and around their local community.

SIL is available for people who live independently in the North Simcoe Community. Staff provide supports with all aspects of daily living.

SLH provide individuals with a high level of personalized support within their CLH DSS homes, which are located across North Simcoe.

The Passport Program provides funding for adults for community participation services and supports, activities of daily living and person-directed planning.

HFP is available for adults to live semi-independently within a family home.

The CNSC – Central East provides coordination and linkages for adults with developmental disabilities with high support and complex care needs, as well as specialized resources for consultation and treatment.
